Sonographic Differential Diagnosis

Inflammatory (supperative, necrotizing, eosinophilic, lymphoplasmacytic, granulomatous, pyogranulomatous); Hyperplasia (infectious, immune-mediated, congestion); Infiltration (neoplasia, extramedullary hematopoiesis, amyloidosis

Image Interpretation

Severe massive splenomegaly having diffusely mildly heterogenous hyperechoic echogenicity measuring up to 8cm wide (n < 2.5cm). No focal lesions appreciated. The spleen is visible in every quadrant of the abdomen.

Outcome

Initial splenic aspirates followed by splenectomy. 8 lb spleen removed and biopsies done.

Sampling

FNA/cytology of the spleen – normal splenic parenchymal cells and marked extramedullart hematopoiesis (EMH) with numerous erythroid precursors and megakaryocytes; no evidence of inflammation or neoplasia on these slides.
