Colonic foreign body, intestinal adhesions in a 5 year old FS Bulldog

A 5-year-old FS Bulldog was presented for vomiting. Evaluation of this dog included recent blood work which showed hemoconcentration, hyperamylasemia, and hyperlipasemia. Survey abdominal radiographs showed no obvious foreign body and no obstructive pattern.

Sonographic Differential Diagnosis

Video 1: stool material vs colonic foreign body;Video 2: Suspected mesenteric adhesions vs acute local inflammatory response caused by the passing of the foreign body

Image Interpretation

Video 1: 6 cm shadowing focal shadowing structure was noted in the descending colon at the level of the urinary bladder. Video 2: In the distal small intestine, multiple omental adhesions were noted associated with the jejunum in the right caudal abdomen.

DX

Outcome

The patient was hospitalized and treated with IV fluids, Baytril, and fed a prescription bland diet. The Baytril was continued for a week and the owner continued to feed the Hills I/D for several months. A re-examination ultrasound was performed following symptomatic treatment and full recovery approximately one month later. The colon was normal in appearance. There was no evidence of mesenteric reactivity or obvious adhesions in the jejunum. On additional follow up, the owner reported that Lola is doing well.

Clinical Differential Diagnosis

GI pathology – Acute gastritis ( viral/bacterial, dietary related, foreign body, toxin), early stage of IBD/gastrointestinal neoplasia, acute pancreatitis, hemorrhagic gastroenteritis.
