DX

The findings are consistent with feline lower airway syndrome (formerly termed asthma).

Image Interpretation

 

 The caudal compartment of the left cranial lung lobe presents a lobar alveolar infiltrate with decreased lung volume. Mid left-sided mediastinal shift is noted.  The cranial compartment of the left cranial lung lobe and the left caudal lung lobe present compensatory over inflation with increased radiolucency.  The remainder of the lung parenchyma reveals a marked generalized bronchial pattern with peribronchial cuffing.  The diaphragmatic cupola is mildly flattened. The lumbodiaphragmatic angle presents increased width. 

A moderate amount of aerophagia is noted throughout the entire gastrointestinal tract. 

Outcome

The compartmental atelectasis of the left cranial lung lobe is due to chronic obstruction of the lobar bronchus by a mucous plug and usually irreversible.
Bronchoscopy with bronchoalveolar lavage may be considered in case the patient is refractory to empirical treatment for feline lower airway syndrome to rule out superinfection.
