Urachal?

Extreme internet introvet here, braving a post. 

One of the other vets in my practice saw this 1 year old Shepherd yesterday. Spayed at 6 months old, and now having a bloody vaginal discharge. Unfortunately she did not have a complete ultrasound, and I only got a couple of minutes with the probe on her. My pictures stink, the exam is not complete, and I’m dying to get her back on the ultrasound table. Really wish I had a video clip! I will certainly understand if you pat me on the head and just call these non-diagnostic.

There is a fluid filled structure ventral to the bladder which seems to wrap around to, and communicate with, the cranioventral aspect of the urinary bladder. 

I’m finding bladder diverticula ultrasound images at http://www.ultrasoundcases.info/Slide-View.aspx?cat=247&case=4807 and some other sites that make me think that this is what we might have here.

The other working theory is stump pyo, but I feel like it is too far cranial (could a stump really stretch that far?), the wall should be thicker, and the contents should be more echoic. 

We may have an exploratory tomorrow to solve the mystery.

Unfortunately difficult to

Unfortunately difficult to intrepret the image but from what you describe it could be a urachus and yes it is a bit cranial for a stump pyometria. Before jumping in surgical, I would recommend that you repeat the US and possibly do a contrast cystogram/retrograde vaginogram.

Yep classic outpocketing in

Yep classic outpocketing in the apex. Surgicate it. Here is a confirmed one just like it> I realized we only had a cat urachocele in the basic search have to work on the sonopath elves to kick in the processing as I know we have some in the lineup:)
