Suspect glomerulonephritis or interstitial nephritis

10 year old FS Westie. Patient presented for increased respiratory rate. Hx of chronic mucous/sneezing. Rhinoscopy has been performed, but unsure of results. Owner thinks may have had mild pre-existing renal disease, but awaiting records. On pred for neck pain. Rads last night showed a mild leukopenia (4100), renal azotemia (BUN 130, creat 3.7, Phos 9.9), elevated globulins 4.5, active urine sediment with a large amount of protein. Rest of labs unremarkable. Also evidence of aspiration pneumonia on thoracic rads.
Kidney appearance to me makes glomerulonephritis or interstitial nephritis my top differentials, but would love clarification. With the medullary rim, should Lepto still be on my list? Reportedly recently vaccinated, but I’m awaiting to see if Lepto included.

Note – won’t let me upload an wmv format video, but I’ve been told my your IT this is ok, so I will e-mail the video to him to see if he can add

Update – Renal values have doubled overnight despite being on IVF

Its anon specific interstitial nephrosis pattern with a medullary rim sign. Dry FIP can do this as well as any toxin exposure. fna to assess granulomatous changes or bx preferred but doesnt sound resolvable if not responding. MP4 video is always best most machines offload in that format but ill check with IT. Thanks for working through the move to a more recent platform our prior was no longer supported and this is a really big job with 40k+ cases in the searchable archive and all the old forum posts but we are getting there

Thank you, Eric!
