To me the L kidney was small with an irregular capsule indicating possible infarcts. It appears that there may be a mass or irregulatrity of the cortex of the caudal pole.
The R kidney looked small – but I did not appreciate some of the capsular or cortex changes.
The R kidey was right at the junction of the 13th rib and abdominal wall. I could not get good images intercostal and there for the R kidney is tipped and not of the greatest quality.
Any input would be appreciated.

Chronic interstitial
Chronic interstitial nephrosis pattern and infacts with multifocal capsular collapse. May have some level of primary dysplasia and these are secondary changes. Would need renal Bx to be sure. Renal dysplasia is a highly variable manifestations with some dying at birth or entering renal failure in adlolescence and others living a relatively normal life til they have renal failure at 8 years old. Can only Bx to know. Chronic lepto can do this as well given the interstitial nephrosis pattern.
