Sounder is an 11 year old F/S Border collie that came for ADR. Bloodwork normal except azotemia ( suspect prerenal)
X-rays showed splenic mass effect. However when we did the US we confirmed splenic tumor but also suspect left limb pancreatitis.
Wondering if splenic tumor was incidental findings and her pancreatitis is the “main” problem? Would you wait on splenectomy and treat her acute pancreatitis while monitoring for bleeding out or just go to exploratory and deal with pancreatitis on recovery…?
Thanks

Or the other possibility is
Or the other possibility is that is a large clot ( negative for doppler) and that this is only a complex splenic mass ?

That is an aggressive sarcoma
That is an aggressive sarcoma type mass bursting at the seams (echogenic fat attached to the mass and spreading regional inflammation…. “pinch a grape” phenomenon I call it. Have to stablize a number of hours wiht iv support then cut if all is resectable.

Tumor is a ticking time bomb
Tumor is a ticking time bomb and the sooner it is removed the better, however, possible manage the pancreatitis a couple of days before doing surery. If tumor ruptures can result in death or else seeding of the abdomen. Do you have a cPl for the patient?, as this may guide you when to do the surgery.
