Scant Vaginal Discharge in Spayed Female

Patient is a 6 year old lab cross, spayed 1 year ago w/out complictions,

Presents w/ small amount of bloody vaginal discharge….Vaginal cytology is just RBC’s and normal, non-cornified epi’s. UA is normal. Vaginal exam finds normal vault but glove is covered w/ darker tingued blood.

U/S: I see something caudal to the left kidney but looks more like a LN to me rather than an ovary???? Pelvic views look normal to me, but on a couple images I can make myself think that I see something dorsal to the bladder that might be a uterine stump. Not fluid filled, but if there is some ovarian hormone influence (although not evident by the vaginal cytology) could the cervix relaxed enough to allow some drainage into the vaginal vault???

I’m not really seeing any obvious uterine remnant pathology that may explain the bloody vaginal discharge. Any vulvar swelling? 

I think I would assess for an ovarian remnant first, even with the non cornified epithelial cells on vaginal cytology. Maybe consider an LH test initially which is, I think, fairly cheap. If the result is questionable, I would suggest a an anti-mullerian hormone test. 

Otherwise, vaginoscopy to the level of the cervix would be suggested to assess for structural abnormalities if the bleeding is persistent.
