Pyloric lesion

– 13year old FS Jack Russel w history of unexplained weight loss, decreased appetite, no vomting

– bloodwork unremarkable

– pet fasted prior to u/s

-stomach distended with fluid and obstruction in the pylorus but with what? polyps, tumour, FB

– the wall looks plicated with digesta caught in between and the muscularis thickened

– rest of GI tract empty and no other abnormal findings

– exploratory recommended

Nioce image set. The caudal aspect of the pylorus has echogenic remodeling and loses mural detail meeting neoplastic criteria and is reminiscent of gastric carcinoma but chronic festering gastritis and fibrosing mural disease or hypertrophic pyloric gastropathy (HPG) can also do this… hence the need for tissue. Needs a billroth but scope may give a dx but need need samples. HPG is usually concentric while this is irregular and unilateral more consistent with carcinoma. I have tru cut these before but have to put the cowboy hat on:)
