Puppy unusual bowel

– 11 week old F Great Dane w history of metronidazole responsive diarrhea

– eating well, growing well no pu/pd;  mild anemia on bloodwork

– u/s showed bilateral pyelectasia with decreased c/m distinction – urine culture pending

– there is an unusual loop of small intetsine shown in the clips just proximal to the ICJ

– there is a region with too many layers that seems to communicate with normal bowel proximally and then transitions into the ICJ distally

– in transverse it looks llike 2 loops of bowel connected in the middle

– is this normal ileum? I  have never seen it look like this before – some sort of intussusception?

Yes the ileum can look like

Yes the ileum can look like that when there is wall thickening. Remember it has the wagon wheel appearance in cross section? This appearance is due to mucosal folding such as the rugal folds in the stomach. Mucosal & submucosal swelling – as in the case here – can easily create the impression of multiple layers because the beam hits the folded wall repeatedly. 

This has nothing to do with an intussusception or malformation. Its just a function of the heavy enteritis going on here. The mucosa is not only swollen but also has an echogenic surface indicative for enteritis with/without protein loss.
