Hi…since my last post on potential PSS shunt was a delusional effect of my excitement about having learnt more on the imaging technique…I would like reasurance on this one…although…I must say I’m pretty convinced this is a PSS. At least, some fine tunning on what my interpretation is of this…

– Female stray DSH cat unknown age, appears adult in teeth and face but looks like a kitten in body shape (2.4kg). EDDU normally, only quite dehydrated.

– Female stray DSH cat unknown age, appears adult in teeth and face but looks like a kitten in body shape (2.4kg). EDDU normally, only quite dehydrated.

– LIver: no much significant hypoplasia, I think. Between right adrenal level and portal vein trifurcation, there appears to be a connecting vessel from PV to CVC with smaller peripheric bypassing neovascularization (that is as much as I can interpret; it looks like a mess to me…). There is no significant CVC distension but I can see turbulent flows. I dont quite see a double aorta, but I could be wrong…

-Also…they asked me to check whether she was spayed or not…for getting her spayed…sigh, i could not find a clear uterus at bladder neck and potentially ovaries might be small of approx 0.4cm…I am thinking there is hypoplasia due to underdeveolpement consequence of PSS?

-I would appreciate your interpretation of these clips.

Well Im seeing color bleeding

Well Im seeing color bleeding between the pv and cvc and normal pv volume and normal intrahepatic vascular volume. I agree there is a tortuous vessel at an odd angle but not sure on the shunt issue here. Trace free fluid. Cats can have normal pv volume which you have shown here but still have a shunt.

If LEs are up this may be early portal hypertension causing the vascular tortuosity. BAs up?

Im thinking more hepatic parenchymal disease here.

Ah! Really?? I thought ir was a clear one!!! Sigh. So… This cat is stray and on a budget. As no bloods were taken Im a bit lost in here… I don’t remember having seen free fluid. My colleague is aware that I love scanning anything, anytime, so she does feed me all her cats for scanning( I manage to squeeze them in …) so I don’t think there will be much investigation in this case… For now it’s just being observed in the clinic, searching for reasons and symptoms for her underdevelopment… There might be some bloods results I’m a aware of. Anyway thanks a lot.
