9 yr MI Samoyed with severe lethargy, pre-renal azotemia (mild), anemia (PCV/TP 30/7.8), evidence of an active urine sediment with some granular casts, current on lepto vaccine. Kidneys look bright to me and prostate large (expected), but more mottled than normal. I am planning to test for Lepto despite being current on vaccine and treat for prostatitis pending results. Thoughts?

The prostate is very large
The prostate is very large with a mixture of poorly marginated hyperechoic and mildly hypoechoic parenchyma with some small cysts. The capsule is rounded yet symmetrical and I dont see obvious inflammation around the prostate. I dont see evidence of mineralization as with neoplasia. If signs of prostate pain on rectal palpation or caudal abdomen palpation, it may be prostatitis. If no evidence of pain or hematuria / pyuria, it may be significsnt BPH. It seems large enough to FNA for cytology and C/S which, I think, would be more diagnostic than prostatic wash and possible contamination.
I agree the renal cortex is bright with medullary mineralization. The kidney looks more chronic than acute and suggest chronic nephritis. I would suggest full urinary workup including urine C/S, UPC if sediment clears and Lepto / infectious disease testing. Blood pressure as well.
