Pancreatitis/IBD?

12 yr old FS DSH with intermittent vomiting and mild weight loss

– eating well most of the time but sometimes picky with food
– one episode of diarrhea
– elevated feline pLI
– biochem, cbc and T4 otherwise normal
– liver, GB and bile ducts normal on ultrasound
– left limb pancreas prominent pancreatic duct and mild reactive fat
What do you think of this pancreas otherwise? Chronic pancreatitis?

What do you think of this pancreas otherwise? Chronic pancreatitis?

– small intestinal walls diffusely thickened with prominent muscularis IBD? Lymphoma?
– no enlarged abdominal LN’s identified
– owners decline intestinal biopsy Considering a low residue diet, B12, metronidazole and possibly a pred trial –

Looks like chronic ibd +/-
Looks like chronic ibd +/- chronic active panc to me jaquie. Or at least the pancreas has been beat up in the past and has coarse parenchyma, undulating capsular contour and duct dilation (DD can be age related in cats). That intestine diff also includes emerging lsa, mct and dry form fip but more likely IBD. ideally shopping spree of intestinal and panc bx but i like a tiral of hypoallergenic geriatric diet, zithromax 50 mg sid to get upper gi bugs and rickettsials, b12 if losing weight and prednisolone as need to deal with clinical signs. Works most the time and if not then suspect the bad boys run the diff and back to the shopping spree idea.

One of my favourite initial drugs for IBD or pancreatitis in cats is Metronidazole. If they don’t regain or stabilize with weight on that, then I usuallly go to Prednisone.

Definitely B12 is good, especially if low in blood.

I’m not a consultant, but spent 10 years working in cat only practices. :). I went through lot of Metronidazole.

I often find it hard to interpret what is normal and what is not when it comes to the pancreas unless there is full blown pancreatitis present – esp in cats. Having an elevated PLI helps.

We get metronidazole compounded into 50mg beef flavoured tablets that are usually easy to give to most cats. Eric, I have never used zithromax for GI disease but have used it alot for feline resp.disease – interesting recommendation!

Don’t know why the combination works but it does and yes metro too mainly for immune modulating component and funky bugs and can combine with zithro. I like zithro because its dis in a liquid and more likely to actually end up in the cat:)
