Next step wtih splenic mass

– bad vet pet owner here, this is may dog “Sparky” a 12-15 yr oMN Jack Russel Terrier I adopted from the shelter anout 4 years ago

– a month ago I did a COHAT on him and pre-op bloodwork showed a mild anemia

– I did a ultrasound on him this week and found a couple of ill-defined hypoechoic nodules in the body of the spleen and a heteroechoic mass in the tail that was distorting the capsule margins

– see cytology report

– Sparky is clinically well and reperat CBC showed an improvement in his anemia although still midly anemic

So – should I run a PARR or just go ahead and remove the spleen and sample the liver and anything else suspicious while I am in there? I realize if lymphoma, splenectomy will not be likely curative

I would fna liver any time

I would fna liver any time spleen is possibly lsa/round cell neoplasia for 2 reasons: 1) To stage to see if just in spleen and surgical or in both spleen and liver and 2) to give th ecytologist more to work wiht if “atypical lymphoid cells” are in both organs he/she is mor elikely to call a definitive dx.

if the liver looks fine and no hepatic ln and no swelling then I would splenectomy and liver bx but if anything in the liver minimally says possible lsa or similar I would fna both again and see what comes up. Image the sternal lns as well to see if crossing the chest.
