I was hoping to ask for opinions on a case. Charles is a 5 year old (rescue so age is unknown) Shi Tzu cross that had an episode similar to syncope 2 weeks ago. Chest rads showed a mildly enlarged La, increased size in the bronchiol vein but no pulmonary edema. He had a grade 5/6 HM

We performed an echo today and he has Mitral valve regurge exceeding 4 m/s and a very thickened anterior leaflet of the mitral valve. No marked increase in the size of the La.

So my question is can syncope still be related to the MVD when there is no major sturctural changes to the heart or is it likely to be unrelated? Can the mitral valve get trapped in the outflow tract and cause syncope? Because the changes in the heart are only mild to moderate B1-B2, should we be looking elsewhere for a cause of the collapse?

MR in a dog should be at least 5 m/sec and your envelop is tailing off oint he first image so lower the frequency or change to a lower frequency probe becaus eyour MR is underestimated. most MR velocities in valve dogs will be 5.5-6 m.sec or higher. Try apical as well.

There is a prolapse here of the mv but the la/ao june boon and la max are normal in video 2 so this is B1 category valve disease. Re the syncope check hypertension and a holter or event monitor… we have them as a service if needed just call the 800 number: 1-800-838-4268 or info@sonopath.com
