Lymphoma versus Pancreatitis

Hello,

 

This is a 8 years old Boxer M/N that presented for chronic vomiting.Normnal PE.  Bloodwork revealed mild increase in ALKP and ALT as well as elevated CpL.

I did find regional pancreatitis in the left limb only which I know is unusual for a dog. I also found enlarged LN : gastric, splenic aortic. Normal right limb pancreas and no enlargment in the mesenteric LN. Stomach wall was subjectivelly thick measuring 0.6-0.9 cm. Rest of the scan was normal.

My differentials are : Emerging Multicentric Lymphoma vs Pancreatitis/ Gastritis with regional lymphadenopathy.

What is your opinion on left limb pancreatitis in dogs with enlarged/ reactive LNs?

Actually left limb

Actually left limb pancreatitis is present more than we thought in the past. See our abstract acvim 2014 http://www.sonopath.com/resources/research-publications

This is most likely a chronic active pancreatitis given the irregular capsular contour and corarse architecture but the rounded Ln bothers me. Certainly 22 g fna on the panc and the LN would help differentiate type of predominant inflammation im guessing lymphoplasmacytic given the chronic changes.

Dont wory about panc fna nothing happens other than getting solid cells:) Ive been stick the pancreas for 18 years and never have had an issue (knock on wood).
