lung and heart: metastasis?

hi all, this is Katie, 10y7m old FN German Shephard with severe weight loss, apathy and on and off diarrhoea.

Abdo scan: regional wall thickening in stomach and spleen appears thickened and mildly enlarged with dis-homogeneous echogenicity and 2 hypoechoic nodules of approx 1cm and 1.6cm dimeter. Small intestines no regional lesions, mild diffuse atypical layering with thickened submucosa. Mild hepatic veins congestion. No free fluid. No respiratory signs.

Thorax: I interprete this findings as “milliary appearance” or micronodules…what are the differentials if not metastatic neoplasia? Could “old lungs” look like this? (i think they wouldn…) I took chest xrays and looked very normal (with only mild bronchial pattern)

Heart: there is a round echogenic shadow inside right ventricle…appears attached to the chordae…thrombus?metastasis?

Patient was negative to heart worm test a while ago and has been on prevention ever since.

The structure in the

The structure in the heart looks to me more like a right pap since it has myocardial echotexture and in located where there are usually 1-2 quite long paps.

I dont see nodules in the lung here. And I would not use ultrasound for ruling out metastasis since you can only see the surface if the lung is not “hepatized”. What you consider nodules is in my opinion artifacts caused by the ribs (am not sure, though).

Thanks Peter, i was wondering if those were papillary muscles. Thanks for the input. I struggled to get a good window for this dog cause I got a lot of lung. Any tips to get rid of lung?
About the lungs… I’m still not sure though, cause the clip is from long axis along the intercostal space, so actually ribs are fully out of the way…
Thank you. I took X-rays for metastasis, but nothing really obvious. I’ll try and post them tomorrow…

Yes I agree on the papillary…The lung looks like normal lung interface to me. When you have mets or lung tumor it looks something like this form the basic search “lung consolidation:”
