Large Feline Kidneys

– 10 year old DSH overweight MN presented for pu/pd but otherwise doing well

– glucosuria found with uirne culture pending – bloodwork pending

– mild hepatomegagly and mild hyperechoic liver echogenicity noted on ultrasound

– kidneys are bilaterally enlarged (over 5cm) with hyperechoic coritices when compared to spleen but I don’t think they look abnormal otherwise and I have seen hyperechoic cortices in cats alot without disease  (fat)

– no renal hypoechoic halo seen, no abdominal lymphadenopathy

Do I worry about these? recommend FNA? monitor via ultrasound?

May be incidental but with PuPd ( I assume that SG is low) and glucosuria it does point to a potential problem. Although the kidneys do not look that bad lymphoma, FIP, and porto-systemic shunts are possible. If blood work is normal, then I would do an FNA of both the kidney and liver.

They have a bit of an

They have a bit of an interstitial nephrosis pattern and 5 cm is big in any category… any food toxin??/ we saw kidneys like this during the food recall a few years ago. FNA 25 gauge screening would be helpful I agree. Big maine coon cat …yeh I can take 4.5-5 cm kidneys if all is normal but they are suspect in any other cat.

We had a similar case recently where a pathologist commented that glucose in a cats urine means they either had hyperglycemia in the last 24 hours or they have some sort of renal tubular damage (Fanconi’s syndrome). It will be interesting to see the BG and renal values as well as electrolytes. Brent

The cat ended up have a blood

The cat ended up have a blood glucose of 20 and normal blood paramaters other than an elevated fPLI so I suspect a degree of pancreatitis even despite a fairly normal looking pancreas on ultrasound. (mildly hypoechoic, normal shape and size)

I do wonder if there is some form of renal insult or subclinical interstitial nephrosis going on.
