gastric neoplasia

Hi all. This is a 3 year old Siberian Forest cat, MN, 3.3kg. Chronic weight loss, good apetite, chronic vomiting. Today started vomiting blood. The clips, in my opinion, meet all the criteria for neoplasia (I´m thinking LSA or gastric adenocarcinoma)…enlarged, distorted LNs, focal large area of gastric wall increased thickeness (up to 3cm!), neovascularization, total loss of mural detail, low motility… I recommended FNA at least or endoscopic biopsy…Probably FNA better to access the external area for a better chance to avoid inflammatory changes?

Question…should this not be a neoplasia…what other differential would we be looking at? (granulomatous??…I´m struggling to think of any other conditions that meet all these findings…in such size…)

Owners are considering the FNA.

Thanks.

This is gastric LSA til

This is gastric LSA til proven otherwise and easily sampled with 22g fna or 25. (http://sonopath.com/members/case-studies/cases/0400406-lillie-c-gastric-lsa)

see the fna here:

On rare occasion I have seen granulomatous gastritis do this… eosinophilic (http://sonopath.com/members/case-studies/cases/gastritis-likely-granulomatous-lesion-11-year-old-fs-persian-cat)… but it tends to be more echogenic. Chase gastric lsa first wiht fna hten if its mixed inflammation go from there with core bx.

Thank you both. I checked the links. I see it actually looks different… As you said. I hope I’m allowed to get a needle in there.
It’s a very sad case… It is a gorgeous, lovely cat… So young…
