– 10 year old MN dsh presented to rDVm August 8th for increased respiratory effort and lethargy BCS 6/6
– chest rads suspicous for mild-moderate pleural effusion
– no heart murmur, bloodwork wnl
– was started on furosemide 10mg TID and pet has improved and post-furosemide chest rads showed clearing of fluid
– patient now currently on 10 mg furosemide BID and doing well

– echo performed today which was really tough to get great images on however I think I got enough to say that this cat likely does not have heart disease as the LV and LAE are normal; mild pericardial effusion is present and I did not see a thoracic mass looking cranially and caudal to the heart through the diaphragm. There was no abdominal effusion present but a complete abdominal scan was not performed (but should have been I know)
I am having trouble calling this pericardial effusion for sure – or can it be a small amount of pleural effusion?
If this is not CHF, then what else? Lymphoma? FIP?
Could lasix therapy shrink the LA down to normal size via volume contraction?

Looks like very small amount
Looks like very small amount of pericardical effusion with normal LV and La. Unless the Lasix has caused severe dehydration, unlikley that the La will shrink to a normal size. Lymphoma and FIP are important differentials, athough with normal blood work FIP less likley. Others would be low-grade septic effusion and chyle. Ideally should have tapped the thorax prior to the Lasix therapy.

I would scan the abdomen looking for primary neoplasia and if nothing check vasculitis/infectious. Doesnt look like cardiogenic effusion to me.
