Esophageal tumor vs foreign body

Hello

We performed endoscopy on a 10 year old GR who ate cabbage rolls with tooth picks 5 days ago. Symptoms waxed and waned. Regular vet performed xrays 3 days ago and found no obvious abnormalities. Continued to gag and has lost her appetite; drooling and seems painful in the mouth. 

Intubation revealed a firm but not hard swelling in the Left tonsilar area. It had none discrete borders and measured and area of 5cm x 2 cm; FNA revealed no fluid and no WBCs; Only RBCs and rare unidentifiable amorphous cells. Submitted anyway.

Endoscopy of the esophagus revealed a round protrusion that seemed to have an elongate associated swelling. Near the thoracic inlet.

The lump is hard and when a biopsy was attempted the endoscopic forceps seemed to slide off of the lump and the esophageal mucosa seemed freely movable over the lump. 

I know this is a stretch but I attempted to post a pic of the lump through my endoscope. I do not have a camera. “Thought my US images were poor!”

The blood is from my biopsy!

I’m not expecting alot of info based on what I can offer but I am also not expecting a diagnosis via cytology. I was hoping for some direction. I think it’s unlikely to be a tooth pick fb in the esophagus.

Sorry. Blood on lump is from endscopic biopsy. Did appear mildly more vascular then the rest of the esophagus.

I dont know how much i can help from here but this lioks like a mural elesion breaking intot he lumen as opposed to a penetrating wound. If you can palpate it try putting a probe on it and we can likely tell more.

Do you see Spirocerca lupi in your neck of the woods?, as that would be an important DDx.

Not that I know of but I should prove the travel history more extensively.  Maybe run a fecal.
