duodenal thickening and ulceration?

10 yr. FS DSH chronic vomiting and 1 lb weight loss in last month. Retches and vomits bile, mostly at night. Overall, overweight cat. Blood work unremarkable. 2-4 week trials of ZD and Microbiome have not made a difference. Responded to maropitant. First posted video (7) is in right lateral and mainly looked like mild irregular filling in the duodenum, but the other duodenal videos that are in dorsal recumbency show what looks like significant mucosal thickening to me and then a little more distal, some gas splitting across wall layers – concerned about ulceration? Am I reading too much in to this? Also thought the pancreas looks like an acute on chronic process? Have recommended duodenal biopsies – is this premature?

There is diseased intestine for sure with reactive mesentery but without neoplastic criteria. Maybe ulceration yes i would tx medically and rescan in 4-6 days or so. The panc doesn’t bother me much the hyperechoic mesentery looks like its associated with the intestine and not necessarily the pancreas.
