Dilated Cardiomyopathy

-Bella is a 6 1/2 year old femal spayed Siamese.

-She was seen toward the end of last week by one of my associates for severe weakness, tachypnea and weight loss. X-rays were taken at that time and it showed Cardiomgally, pleural effusion and pulmonary edema. My associate hospitalized her and administered oxygen and Salix. She set her up for an Echo with me on Tuesday of this week.

– I had to share a few images with the Sonopath community. I have not seen a Dilated Cardiomyopathy for some time.

– Rx at this time consists of Salix 12.5 mg q12h, Vetmedin 5 mg (1/2 tablet q12h). This is a bit high- but I was out of the 1.25 mg size tablets and the calculated dose was 1.87 mg q12h.

– Taurine tablets – 1/2 tablet q12h.

Recheck is scheduled for next week at which time I may start Enalapril and Plavix. No smoke was seen or clots at this time. 

Any further suggestions would be appreciated. I am surprised this cat is still alive.

This is a typical case of feline DCM. I totally agree.

In cats with DCM I usually measure the Taurine level and start with Taurine immediately while the result is pending.

Regarding Vetmedin: I would rather give 3/4 of a 2.5 mg pill given your calculated dose. An ACEI and Plavix sound reasonable to me as well, as you mentioned.

These cats, if not taurine-dependent, usually live a couple of weeks to a few months…

If this cat responds to the Taurine- would this be lifetime therapy or can they be weaned off the medication?

This cat was on Purina OM- have there been any indications in the literature that this food may be deficient (hard to believe)?

I seem to be seeing a lot

I seem to be seeing a lot more cats with DCM without any apparent nutritional deficiency.

Is this similar to forum members?

Update: unfortunately Bella passed away this week. Not suprising considering the FS was at 10%
