– 12 year old F Sheltie presented for urine leaking and protruding vaginal mass
– ultrasound showed enlarged uterus body, and both horns with severel mural cysts and gas artificat (infection?); the uterus was not distended as would be seen with a pyometra
– both ovaries had cysts present; urinary bladder, trigone and proximal urethtra normal
– the protruding vaginal mass had a smooth, regular echotexture with low vasculairity on colour Doppler
– left medial iliac LN enlarged (hyperplastic vs neoplastic)

My main differential would be cystic endometrial hyperplasia (neoplasia low potential) with possible concurrent metritis due to the gas reverberation seen in the uterus. The vaginal mass may be a hormone-responsive leiomyoma. OVH and vaginal mass removal recommended and histopath.
Wondering if anyone has seen this presentation? I don’t scan many intact female dogs.

looks like gas echos in the uterine wall… Ive seen this with open pyo or at least metritis which is what I would expect if any discharge as opposed to closed that dilates the tubes. Bottom line needs OVH and abs with culture & bx of uterine wall. When repro goes south needs to come out:)
Sounds like the vaginal
Sounds like the vaginal protrusion is a form of hormonal vaginal hyperplasia. Like EL said OHE should be curative for all the issues.
