DX

Left adrenal mass with regional iliac lymphadenopathy and splenic nodules; Pheochromocytoma suspected

Image Interpretation

A left adrenal mass was noted with capsular expansion, measuring 5.6 cm x 3.7 cm. The left adrenal mass adhered against the aorta and wrapped around the left renal artery. It would be extensively difficult to resect. The vena cava appears clean. However, the aorta and left renal artery are enveloped by the mass.

The spleen revealed a focal hypoechoic expansive nodule, measuring 1.5 cm x 1.2 cm at the caudal pole. The spleen presented other micronodular changes. Splenic thrombus noted within the splenic parenchyma of the main splenic vein, expanded approximately 1.0 cm. Slight areas of free fluid were noted adjacent to the spleen.

Sampling

25 gauge FNA of adrenal mass and spleen performed without complication
