A 10-year-old female spayed German Shepherd Dog with a history of adenocarcinoma of the anal sac was presented for evaluation. Survey radiographs were suspicious for metastasis to the sublumbar lymph nodes.

DX

Suspect medial iliac LN metastasis from ASA

Sonographic Differential Diagnosis

Iliac lymphadenopathy. The parenchymal portion of the iliac lymph node is concerning. Recommend assessment of the aspirate results. However, given the patient’s history iliac spread from adenocarcinoma is suspected.

Image Interpretation

The iliac lymph node was enlarged, hypoechoic and irregular at 5 x 7.42 cm. Given the patient’s history, this is strongly suggestive for metastatic disease. The right iliac lymph node was filled with fluid; however, significant parenchymal portion of the lymph node remained. This is still suggestive for infiltrative disease.
