Suspect interstitial cystitis in a 2 year old MN DMH cat

A 2-year-old neutered male DMH cat was presented for urethral obstruction and a possible mass dorsal to the urinary bladder. The cat also had a 5-day duration of diarrhea.

A 2-year-old neutered male DMH cat was presented for urethral obstruction and a possible mass dorsal to the urinary bladder. The cat also had a 5-day duration of diarrhea.

Sonographic Differential Diagnosis

Concentric bladder wall thickening. This is consistent with aggressive interstitial cystitis. There is a small potential for bladder lymphoma or other neoplasia.

Image Interpretation

The urinary bladder in this patient presented a concentric thickening of the trigone and ventral cystourethral junction continuing into the urethra. The bladder measured 0.71 cm with echogenic debris. Significant, periserosal inflammatory pattern was noted. A minor amount of sand and debris was noted in the urinary bladder with suspended debris as well. The apex of the bladder appeared unremarkable. The mural pathology appears to be concentrated in the cystourethral junction and ventral and dorsal caudal aspects of the bladder.
The kidneys revealed normal size and structure, corticomedullary definition and ratio (cortex 1/3 of medulla). The cortices presented largely uniform texture with normal echogenic relationship to liver and spleen. Medullary echogenicity differed distinctly from that of the cortex and no evidence or dilation could be seen. The capsules were acceptably uniform without dramatic irregularities. The right kidney measured 3.7 cm. The left kidney measured 3.82 cm.

Clinical Differential Diagnosis

Bladder disease – interstitial cystitis, uroliths, bacterial cystitis, trauma. Possible mass – neoplasia, lymphadenomegaly (reactive, adenitis, lymphoma), abscess, hematoma.
