String foreign body and lymphoplasmacytic, eosinophilic enteritis in a 14 year old FS Dachshund

A 14 year old FS Dachshund dog was presented for vomiting. The patient was treated with Pepcid, dexamethasone, Cerenia, and a bland diet. The owner called once stating that there was still ongoing vomiting, and that he was concerned that the vomiting was due to the steroids. Blood work, abdominal radiographs, and abdominal ultrasonography were recommended if there was no improvement.

Sonographic Differential Diagnosis

This is most consistent with an obstructive pattern either due to dysfunctional bowel or embedded foreign matter.

Image Interpretation

Mild residual ingesta was noted with some partially shadowing material in the upper small intestine. This material continued into the mid small intestine, which was followed by empty aperistaltic small intestine.

DX

String intestinal foreign body, lymphoplasmacytic and eosinophilic enteritis

Outcome

On laparotomy, string-like material was removed, and an intestinal biopsy taken from the jejunum. Urine cortisol:creatinine ratio and ACTH stimulation were within normal limits.

Clinical Differential Diagnosis

GI tract disease – infection, obstruction, neoplasia, IBD. Liver disease, pancreatitic disease, renal disease.

Sampling

Surgical intestinal biopsy taken from the jejunum showed moderate lymphoplasmacytic and eosinophilic enteritis with moderate edema.
