Stricturing colonic mass in a 14 year old MN DSH

Case Study

Stricturing colonic mass in a 14 year old MN DSH

A 14-year-old NM DSH was presented for evaluation of chronic diarrhea and weight loss. Abnormalities on CBC and serum biochemistry were mild anemia, elevated liver enzyme active (AST 184, ALT 163) and mild hyperglycemia.

Image Interpretation

A stricturing colonic mass was noted in the descending colon. Wall thickness measured 1.0 cm serosa to mucosa with complete thickening of 2.5 cm. Minor intestinal thickening was noted elsewhere. The colonic infiltrative pattern extends into the pelvic inlet beyond the level of the cystourethral junction. Therefore, resectability is difficult.

Outcome

Splitting the pelvis would be necessary for surgical resection. Ultrasound-guided FNA of the wall of the colon is recommended to assess for carcinoma versus lymphoma or a mild potential for granulomatous disease. Colonoscopy is another option. Guarded prognosis.

Clinical Differential Diagnosis

Hyperthyroidism
GIT – inflammatory bowel disease, dietary hypersensitivity, neoplasia, foreign body
Liver – cholangio-hepatitis complex, neoplasia, granulomatous disease
