Image Interpretation

The stomach and upper gastrointestinal tract was unremarkable. The descending colon in this patient was significantly thickened with loss of detail, a stricturing pattern and stool obstruction. Focal areas of mineralization were noted within the colonic pathology. The pathology was embedded within the pelvis and not overt resectable without splitting the pelvis. Iliac lymph node was enlarged at 2.4 cm, yet the length to width ratio was maintained.

DX

Stricturing colonic lesion with iliac lymphadenopathy

Outcome

Colonic carcinoma versus chronic colitis and secondary stricture is possible. Ultrasound-guided FNA of the colonic wall thickening and lymph nodes would be recommended with appropriate culture and cytology.
