Splenomegaly with micronodularity in a 6 year old MN ferret

Case Study

Splenomegaly with micronodularity in a 6 year old MN ferret

The patient is a neutered male 6 year old ferret. The patient was presented for cough and palpable enlarged mesenteric root lymph nodes. White cell count was subnormal at 4.1 and hematocrit slightly subnormal at 38.

Sonographic Differential Diagnosis

This scan is strongly suggestive for splenic lymphoma with regional lymphadenopathy (epigastric, mesenteric root and hepatic), with possible hepatic involvement. Although ferrets often have a typical hypersplenism where the spleen is uniformly enlarged, in this case, the focal expansion of multiple lesions is more suggestive of neoplasia.

Image Interpretation

The spleen was enlarged, moth eaten and micronodular. Multiple, aggressive, expansive nodules were noted throughout the spleen, which is strongly suggestive for lymphoma or similar neoplasia. There was a moderate amount of hepatic remodeling with increased portal markings. A slight amount of free fluid was noted cranial to the liver. Cranial abdominal lymph nodes were enlarged and measured 1-1.5 cm in the epigastric region as well as the hepatic region. The mesenteric lymph nodes were also enlarged.

DX

Splenomegaly with micronodularity, abdominal lymphadenopathy, and suspected lymphoma

Outcome

None

Clinical Differential Diagnosis

Neoplasia, FIP-like syndrome, lymphoreactive disease, infectious.

Sampling

25-gauge to 27-gauge FNA of the spleen, lymph nodes and liver would be recommended for a definitive diagnosis but was not completed.
