Case Study

Small intestinal obstruction with annular mass in a 14 year old MN DSH cat

This 14 year old MN DSH cat has a history or stable renal disease. Presentd with intermittent vomiting of 2-3 weeks duration.

Chem: BUN 48, Crea 3.4

Image Interpretation

Both kidneys revealed presented loss of corticomedullary definition. The left kidney measured 3.56 cm with pericapsular inflammatory pattern and pyelectasia with echogenic debris. This is suggestive for pyelonephritis. Full urinary work-up would be warranted. The right kidney revealed pyelectasia as well. The right kidney measured 3.01 cm.  The stomach was filled with ingesta. Distal small intestinal mass was noted and was obstructive in nature. An annular mass was noted at the level of the ileocecal junction. This created significant obstruction. This appears potentially resectable.

Outcome

Exploratory surgery is recommended with intraoperative ultrasound to delineate the
infiltrative pattern within the intestine. The intestinal mass measured approximately 2.0
cm in width with approximately 6-8 cm of infiltrative pattern. Carcinoma versus
lymphoma are the primary differentials. Chronic inflammatory intestinal disease is also
possible, yet less likely.
