Renal mass in a 10 year old MN DSH cat

A 10-year-old NM DSH was presented for evaluation of acute onset vomiting, vocalizing, and abdominal pain. On survey radiographs, a  possible mass or left renomegaly was evident. Blood work was unremarkable. Prior to the ultrasound, the patient developed pyrexia.

A 10-year-old NM DSH was presented for evaluation of acute onset vomiting, vocalizing, and abdominal pain. On survey radiographs, a  possible mass or left renomegaly was evident. Blood work was unremarkable. Prior to the ultrasound, the patient developed pyrexia.

Sonographic Differential Diagnosis

The left renal mass is most consistent with a sarcoma or other round cell neoplasia. The mass appears isolated. There was no overt evidence of metastatic disease.

Image Interpretation

The left kidney contained an expansive parenchymal and microcystic mass, deriving from the cranial pole and measuring 4.27 x 4.0 cm. The left kidney itself measured 6.2 cm including the recognizable portion of the kidney. The right kidney was structurally unremarkable except for a minor interstitial nephrosis pattern. Slight, cortical collapse of the caudal pole was noted, likely due to an infarct. The right kidney measured 3.84 cm.

Clinical Differential Diagnosis

Renal disease: neoplasia, abscess, hydronephrosis, pyelonephritis; Abdominal mass: neoplasia, cyst, abscess or granuloma of any of the following – spleen, pancreas, mesentery, or lymph node.

Sampling

None. Immediate left nephrectomy is recommended.
