Renal lymphoma

Sonographic Differential Diagnosis

Strongly suggestive for renal lymphoma.

Image Interpretation

The right kidney measured 3.86 cm with mild degenerative changes and pericapsular fluid accumulation noted. Pyelectasia of the left kidney was noted and measured 0.54 cm with cortical infarcts, moderate remodeling, and dystrophic mineralization. Both kidneys showed a subcapsular halo or capping (hypoechoic area below the capsule). This measured 1.4 cm. The left kidney itself measured 3.84 cm. The left kidney was completely distorted. Loss of detail and pericapsular inflammatory pattern was noted in both kidneys. Regional renal lymph node was also enlarged and measured 1 x 0.5 cm.

Clinical Differential Diagnosis

Palpable mass: neoplasia, abscess, granuloma or cyst of an internal organ or lymph node; Hydronephrosis.
Anemia: blood loss, immune-mediated, bone marrow suppression/failure. Urinary disease: bacterial cystitis, neoplasia, urolith, pyelonephritis.

Sampling

Ultrasound-guided FNA of the left kidney was performed without complications, and revealed lymphoma.
