RAD – mild interstitial infiltrate of lung with hypovolemia and microcardia in a 6 year old MN Labrador Retriever

This 6 year old MN Labrador Retriever dog presented with coughing a vomiting 2 weeks ago, vomiting resolved but is coughing. Stones on rad in pylorus no longer present.

DX

Mild interstital infiltrate of the lung with some evidence of pleural thickening • Hypovolemia with microcardia and underperfusion of the lung

Image Interpretation

Rads of the thorax – A mild generalized unstructured interstitial pattern is noted emphasizing the right
caudal and left cranial lung lobes. Occasional increased opacity of the bronchial walls
is noted. Multifocal small irregular shaped mineral opacities (ditzels) compatible with
pulmonary osteomas or pleural plugs are noted throughout the lung.
The caudoventral mediastinal reflection is prominent. The cardiac silhouette is upright and thin. The caudal vena cava and the pulmonary
vasculature are thin.

Outcome

The radiographic examination of the thorax does not reveal specific findings
underlying the current clinical presentation.
Even though the interstitial pulmonary infiltrate carries the potential of the entire
spectrum of infectious pneumonia – including viral, bacterial and fungal – the potential
of bacterial, mycobacterial and fungal microorganisms being the underlying issue here
is rather low because of the mild degree of changes and the lack of mediastinal lymph
node involvement.
The potential for this being neoplastic is also considered very low. However, screening for fungal microorganisms is indicated since some fungi species –
such as histoplasmosis spp. – tend to present with multifocal mineralized granulomas,
which is a possible differential of the pulmonary osteomas here.
Bronchoscopy with BAL should be considered for further definition in case the
pending test results are equivocal and the patient is refractory to symptomatic
treatment.
