RAD – mid abdominal mass effect with likely mesenteric pathology in a 13 year old FS Pitbull

This 13 year old FS Pitbull presented ADR, not eating, difficulty walking or even moving, enlarged axillary lymph node, blood in urine (history of UTI), dehyrated.

CBC/Chem: BUN 227, Crea 7.1, Phos 15.9, ALB 4.4, ALKP 4396, Amyl 1622, Lip 5539TT4 0.7 low

U/A: USPG 1.009, WBC tntc, cocci tntc, rbc tntc, protein +3, blood +4

Image Interpretation

Rads of the abdomen –

Possible mildly mass effect and reduced serosal detail within the mid-abdomen. The small intestinal loops are crowding within the right abdomen. The small intestinal loops are of even diameter and non-dilated, but filled with gas and turgid.  Mildly gas distended and rigid duodenum.  The stomach is mildly distended with gas.

DX

Possible mid-abdominal mass effect and reduced serosal detail

Outcome

a full abdominal ultrasound is recommended for further definition and to rule out peritoneal effusion as well as intestinal, mesenteric lymph node, pancreatic or other mesenteric pathology. Diffuse infiltrative hepatopathies such as vacuolar hepatopathy, or less likely a diffuse inflammatory/neoplastic infiltrate are a potential. With the history of elevated liver enzymes and the pot-bellied appearance of the abdomen Cushing’s with steroid induced hepatitis is a possibility to consider as well.
