Pyelonephritis and interstitial nephrosis in a 16 year old FS cat

A 16-year-old spayed female cat was presented for evaluation of hematuria and inappropriate urination. On urinalysis, an inappropriate specific gravity (1.014) and hematuria were evident. The only abnormality on blood work was mild anemia.

Sonographic Differential Diagnosis

Chronic interstitial nephrosis pattern with medullary rim sign and pyelectasia. This is consistent with pyelonephritis.

Image Interpretation

Ultrasound of the kidneys revealed an interstitial nephrosis pattern with a significant hyperechoic medullary rim sign and ill defined renal pelvis (long axis images). Pyelectasia was noted in the kidneys and measured 0.48 cm in the left kidney and 0.38 cm in the right kidney. The right kidney measured 4.03 cm. The left kidney measured 3.8 cm.

None

Comments

Treatment for pyelonephritis is recommended over the next 6 weeks with Enrofloxacin. Blood pressure measurements are also warranted with appropriate renal diet. Recheck culture is recommended at 4 weeks prior to stopping antibiotic therapy. Recheck ultrasound is recommended at that time as well. Antibiotics should be continued for at least 10-14 days post negative culture given the chronic changes in the kidneys. Guarded long term prognosis. 

Clinical Differential Diagnosis

Bladder disease (interstitial cystitis, neoplasia, uroliths, polyploid cystitis, granulomatous cystitis), urethral disease (urolith, neoplasia), renal disease (pyelonephritis, renolith, neoplasia, chronic kidney disease).
