Primary renal dysplasia in a 2 year old MN DSH cat

A 2-year-old MN DSH with history of azotemia was presented for increased thirst, decreased urination, and weight loss. Physical examination showed the patient to be quiet, alert and responsive. The rest of the examination was unremarkable. Urinalysis revealed a low-normal specific gravity and 1+ hematuria. CBC was within normal limits whereas marked azotemia was present on serum chemistry. Blood pressure measurements were high normal.

Sonographic Differential Diagnosis

Images are suggestive for primary dysplasia. Very guarded prognosis.

Image Interpretation

The kidneys presented severe dystrophic changes with left kidney measuring 3.3cm and the right kidney measuring 3.3cm. Corticomedullary definition was poor. The cortices were significantly thickened and irregular suggestive for primary dysplasia. Minor areas of mineralization were noted. Chronic degenerative disease is also noted. Ultrasound guided biopsy is indicated to assess the possibility of primary dysplasia.

Outcome

The patient was discharged with K/D diet and plan to recheck blood work within 3 months. Several months later the owner reported that the patient had passed away.
