Pancreatitis with possibility for pancreatic carcinoma in a 1 year old MN DSH cat

A 1-year-old neutered male DMH cat was presented for evaluation of pyrexia, vomiting, and anorexia. Serum biochemistry revealed elevated cholesterol (284) and hyperglycemia (246).

Sonographic Differential Diagnosis

Aggressive pancreatitis or potential pancreatic carcinoma are both possible in this case. Lipidotic appearing liver, with potential underlying neoplasia, is also noted.

Image Interpretation

The pancreas was enlarged with undulating contour and hypoechoic parenchyma with pericapsular inflammatory pattern of surrounding omentum. There is a small amount of anechoic free fluid in the gallbladder fossa. The liver is hyperechoic to the falciform fat.

Comments

Toxoplasmosis and Bartonella titers are indicated. Plasma expanders, aggressive pain management and broad spectrum antibiotics are warranted. This has a guarded prognosis.

Clinical Differential Diagnosis

Pancreatic disease: pancreatitis; GI disease: obstruction with focal perforation, neoplasia, IBD; Infectious disease: FIP, pyothorax, FIV/FeLV, bacterial peritonitis, abscessation.

Sampling

25-gauge FNA of the pancreas and liver are strongly recommended in this case to rule out neoplasia and define any inflammatory elements for precise therapeutic intervention. No sampling was completed.
