Ovarian cyst and segmental endometrial squamous metaplasia in a 13 year old FI DSH cat

A 13 year old DSH was presented for evaluation of abdominal distension and anorexia. On survey radiographs small intestinal wall thickening, mid abdominal mass, abnormal shape to the urinary bladder, and a small liver were evident.

Sonographic Differential Diagnosis

Pyometra, potential mummified fetus or neoplasia, early peritonitis.

Image Interpretation

The uterus is fluid-filled with mildly thickened wall. The fluid is echogenic suggestive for blood or inflammatory components; the base of the uterus contains a rounded structure which may indicate fetal mummification or a mass. Hyperechoic surrounding reactive fat and localized free fluid indicate peritonitis. The bladder appears normal.

DX

Endometritis, myometritis, and adenomyosis

Outcome

Post ovariohysterectomy the patient recovered uneventfully.

Clinical Differential Diagnosis

Abdominal mass – neoplasia, abscess, organ (such as kidney, spleen, intestine, lymph nodes, or uterus). Intestinal disease – IBD, neoplasia.

Sampling

An ovariohysterectomy was done. On histopathology, an ovarian cyst and segmental endometrial squamous metaplasia with endometritis, myometritis, and adenomyosis (ectopic glandular tissue within the muscle) was diagnosed.
