Case Study

Multifocal diffuse neoplastic infiltrate, likely round cell, involving spleen, pancreas, liver, small intestine and lymph nodes in a 14 year old MN DSH cat

This 14 year old cat presented with weight loss.

Physical exam: alveolitis at both upper canines, unthrifty, incteris

CBC/Chem:   ALT 630, AST 209, ALP 315, bilirubin 8.9

This 14 year old cat presented with weight loss.

Physical exam: alveolitis at both upper canines, unthrifty, incteris

CBC/Chem:   ALT 630, AST 209, ALP 315, bilirubin 8.9

Image Interpretation

Abdominal US – The spleen reveals moderate generalized enlargement with an overall decrease in
echogenicity and a nodular pattern. The splenic capsule is expanded with scalloping
margins. The liver presents mild generalized enlargement with an overall increase in
echogenicity and heterogeneity. Lobar swelling is noted. The pancreas is enlarged – emphasizing the left lobe – with an overall decrease in
echogenicity and a nodular pattern as well as mild loss of echotexture. Mild structured
mesenteropathy is noted in the periphery of the left lobe. The medial iliac and splenic lymph nodes are enlarged and hypoechoic with
surrounding cellulitis/mesenteropathy as well as a short-to-long-axis ration of > 0.5.

DX

Splenomegaly • Hepatomegaly • Nodular pancreas with loss of echotexture • Diffuse infiltrative disease small intestine • Lymphadenomegaly The ultrasonographic findings are suggestive for a multifocal diffuse neoplastic infiltrate such as round cell neoplasia (mainly lymphoma or mast cell tumor) involving the pancreas, spleen, liver, small intestine as well as mesenteric and parietal lymph node
