Sonographic Differential Diagnosis

Mesenteric lymphadenopathy. Infiltrative splenic and hepatic pattern. Multi centric lymphadenopathy.
This is suggestive of multi centric lymphoma or similar neoplasia.

Image Interpretation

The spleen was enlarged with micronodular changes. An overt 2.3 cm mass was noted at the mid caudal body.
The liver was nodular and irregular in contour. The gallbladder was thickened with echogenic debris. Hepatic lymphadenopathy was noted with focal areas of capsular swelling and nodular changes with loss of parenchymal detail. Sublumbar lymphadenopathy was noted. The gastrointestinal tract revealed a 2.0 cm jejunal mass. The upper gastrointestinal tract revealed variable thickening. Midabdominal lymph nodes were also enlarged. Reactive fat was noted throughout the cranial abdomen. The mesenteric lymph node measured approximately 3.0 cm.

DX

Multicentric lymphosarcoma

Outcome

None

Clinical Differential Diagnosis

Elevated liver values: acute hepatopathy (viral, bacterial, toxins), neoplasia, vacuolar hepatopathy, reactive hepatopathy; Gall bladder disease: cholecystitis, liths, mucocele; Pancreatitis.

Sampling

Ultrasound-guided fine needle aspirates of the mesenteric root lymph nodes, the intestine, and the spleen were performed without complication. All results were consistent with lymphoma.
