A 1-year-old intact female dog that had been mated approximately 6-7 weeks ago was presented for evaluation of vaginal hemorrhage. 

A 1-year-old intact female dog that had been mated approximately 6-7 weeks ago was presented for evaluation of vaginal hemorrhage. 

Sonographic Differential Diagnosis

Likely metritis, placentitis and fetal death at the base of the uterus. One of four feti was still alive. The gestation is too early for C-section and is most consistent with approximately 5 weeks of gestation. Treatment for metritis and placentitis would be recommended.

Image Interpretation

The cervix was mildly thickened with a slight amount of fluid. Fetuses were present approximately 4.0 cm from the cervix. There were either three or four feti. Fetus number one had no demonstrable cardiac activity. Fetus number two had cardiac activity. Fetus number three had no demonstrable cardiac activity and some early degeneration of the tissues were present. Fetus number four’s viability can not be assessed as only one still image was sent for review. Mummification process has likely started in these feti.

Comments

Recommend aggressive antibiotics and medical support. However, it is debatable on whether the remaining fetuses will remain viable in order to bring the patient to term. Uterine culture is recommended to assess for bacterial involvement and specific antibiotic therapy. Clavamox or similar antibiotic can be utilized in the meantime.

Clinical Differential Diagnosis

Uterine disease – aborting (fetal death, mummification), metritis, pyometria. Bladder disease – cystitis, uroliths.
