A 9-year-old MN Corgi that had shown vomiting and left-sided abdominal pain was presented for evaluation of a firm and sensitive area on the left side of the abdomen. Survey radiographs showed an abnormal appearing density on the left side.

Left renal mass or abscess with fluid leakage into the retroperitoneal space.
The right kidney was uniform and measured 5.91 cm. The left kidney in this patient presented a mass that was deriving from the dorsal caudal renal cortex with pericapsular inflammatory pattern. There is a potential that this is a cortical abscess. This is not necessarily neoplastic in nature. The corticomedullary junction was significantly disrupted by the pathology. The mass or abscess appears to be expanding caudally with localized, retroperitoneal free fluid and regional inflammation.
The recommendation is immediate exploratory surgery with left nephrectomy. The hope is that this is abscessation and not neoplasia. However, neoplasia such as hemangiosarcoma, which is the primary differential, or other sarcoma/carcinoma are the also potentials as well as abscessation. Ultrasound-guided FNA could be considered to assess if this is abscess. However, this a surgical case.
