Left adrenal cortical adenoma in an 8 year old MN mixed breed dog

Case Study

Left adrenal cortical adenoma in an 8 year old MN mixed breed dog

An 8-year-old NM canine mix was presented for abdominal ultrasound to evaluate the adrenal glands, after a hormonal panel (University of Tennessee) was indicative of Cushing’s disease. Pre-and post ACTH stimulation cortisol, androstenedione, estrodiol, progesterone, and 17OH Progesterone were all elevated, Aldosterone was within normal range. Urinalysis showed a low specific gravity (1.017).

Sonographic Differential Diagnosis

Resectable left adrenal gland tumor. Rule outs include adenoma and adenocarcinoma, which is likely functional given the adrenal panel results.

Image Interpretation

The left adrenal gland in this patient presented a large cranial pole. It measured 1.53 cm in width. The left adrenal gland revealed echogenic changes and loss of corticomedullary definition. The length of the left adrenal gland measured 3.4 cm. The nodule itself measured 1.61 x 1.66 cm. The appearance of the nodule is consistent with adenoma or adenocarcinoma. This tumor does appear resectable. The right adrenal gland measured 2.9 x 0.62 cm.

DX

Adrenal cortical adenoma.

Outcome

Adrenalectomy was performed in this patient.

Comments

No video is available in this patient.

Clinical Differential Diagnosis

Pituitary dependent hyperadrenocortism, functional adrenal neoplasia (adenoma/adenocarcinoma/pheochromocytoma)

Sampling

A surgical biopsy of the adrenal mass revealed resected cortical adenoma.

UA Specific Gravity Range

1.017
