DX

Jejunal foreign body with obstruction

Sonographic Differential Diagnosis

Obstructive mid small intestinal pattern with fluid dilation in the stomach and duodenum. Chronic IBD gastrointestinal pattern. Suspect hairball movement or fabric type foreign matter.

Image Interpretation

The stomach in this patient was severely dilated with fluid as was the upper duodenum. The ileocecal valve was free of evident pathology. The jejunum was obstructed with progressively shadowing material. This is consistent with hair or similar material. Dilated small intestine was followed by empty small intestine was noted. Variable small intestinal wall thickening was noted. This is likely consistent with underlying inflammatory bowel disease. Intestinal biopsies are recommended at the time of surgery.

Outcome

Surgical exploratory and intestinal biopsies are recommended.

Clinical Differential Diagnosis

Toxins, dietary indiscretion, foreign body, viral/bacterial infection
