Jejunal foreign body in a 15 year old MN DLH cat

The patient is a 15 year old neutered male DLH cat. Clinical findings include anorexia and vomiting since for several days. Urine specific gravity was 1.027 with 1+ protein.

 

Sonographic Differential Diagnosis

Thickened distal small intestine with obstructive pattern and intestinal foreign matter. This is likely hair or similar material. Regional omental inflammation and wall thickening is present. It is likely that the hair type material is trapped in the intestine due to focal, dysfunctional bowel. Biopsies or resection of the intestine is recommended at the time of surgery. Spontaneous necrosis, complicated inflammatory bowel, mast cell disease and lymphoma are all possible regarding the intestinal wall presentation. Guarded prognosis.

Image Interpretation

The stomach in this patient was moderately dilated into the pylorus and upper duodenum. Small intestinal obstructive pattern was also noted. A focal area of intestinal wall thickening was noted with retention of progressively shadowing type material. This is consistent with hair or soft foreign matter. The obstructive area appears to be jejunum. However, concurrently concentric wall thickening was noted and measured 0.5 cm with some loss of detail and regional fatty inflammation.
