This 9-year-old MN mixed-breed dog was presented for anorexia, diarrhea, and weight loss. Clinical exam revealed mildly poor body condition and mild dehydration. The CBC evidenced mild leukocytosis with a left shift and moderate hypoalbuminemia on blood chemistry evaluation.

Sonographic Differential Diagnosis

Differentials include inflammatory disease such as gastroenteritis or inflammatory bowel disease and neoplasia, possibly round cell in origin.

Image Interpretation

This segment of small bowel is mildly thick walled, with a prominent mucosal layer. In the far field, an ovoid, 2-cm, markedly hypoechoic lymph node is present. In the mesenteric root, a large, dramatically hypoechoic lymphadenopathy is present. Video 1 reveals the relationship of the enlarged nodes with the mesenteric vasculature. Video 2 demonstrates a 16ga automated core biopsy of the affected node obtained by minimizing the distance between the body wall and target node as well as ensuring a trajectory angled away from the mesenteric vasculature to avoid significant bleeding complications.

Outcome

The patient went into partial remission with a Wisconsin protocol and was relatively asymptomatic for 3 months. He was euthanized at 3-months post diagnosis due to the development of a retrobulbar mass.

Comments

Core biopsy was selected as opposed to FNA in order to adequately diagnose granulomatous disease if neoplasia was not present.

US-guided 16-ga intestinal and lymph node biopsies revealed intermediate-grade lymphoma.
