A 13-year-old F Yorkshire Terrier dog was presented for polyuria and polydypsia and a potbellied appearance. SAP was increased, and her urine specific gravity was 1.012.

Sonographic Differential Diagnosis

Pituitary-dependent hyperadrenocorticism. Bilateral adrenal hypertrophy, right phrenic thrombosis.

Image Interpretation

Bilateral adrenal hypertrophy is present with mild heterogenous parenchymal changes. No capsular escape is noted with uniform swelling. The right phrenic vein is occupied by a tissue thickening consistent with phrenic thrombosis. Minor potential for right adrenal neoplasia.

DX

Hyperadrenocorticism

Outcome

The patient was started on Trilostane. Systolic blood pressure was initially 220.

Comments

The patient was lost to follow up.

Clinical Differential Diagnosis

Cushing’s disease, pituitary or adrenal. Diabetes insipidus, psychogenic polydypsia, UTI and medullary washout. Increased SAP – hepatopathy (steroid, diabetes, age), liver neoplasia.

Sampling

ACTH stimulation test was consistent with hyperadrenocorticism (Cushing’s disease).
