Hepatocellular carcinoma in a 9 year old FS DLH cat

A 9-year-old FS DLH cat was presented with anorexia. Physical examination revealed mild dehydration and poor body condition. Mildly elevated SAP and mildly elevated ALT were found in the blood chemistry profile. Urinalysis was unremarkable.

Sonographic Differential Diagnosis

Hepatic abscessation or necrotizing neoplasm occasionally present with similar characteristics and cannot be ruled out.

Image Interpretation

Long-axis view of the right liver demonstrates a well-circumscribed, ovoid, echogenic, multicystic right lobar liver mass impinging on the diaphragm. The echogenic appearance is the result of distal enhancement within the numerous small, closely associated cysts comprising the mass.

Hepatocellular carcinoma

Outcome

Surgery was performed in this case because the pathology was localized to the right lobes, and the adjacent portal hilus was not affected. The patient recovered reluctantly owing to postoperative anemia but remained asymptomatic for 5 months until a mass developed in the cranial mediastinum. The cranial mediastinal mass was not defined histopathologically but was aspirated to reveal lymphoid hyperplasia. The owner elected humane euthanasia at that time.

Clinical Differential Diagnosis

Multi-organ pathology: pancreatitis, neoplasia, inflammatory hepatopathy, biliary calculi, IBD, GI obstruction.

Sampling

Surgical resection and biopsy revealed hepatocellular carcinoma.
